bq24296的SYS脚充电瞬间被击穿

bq24296RGER，10个样板。其中两片接电池可正常使用，但在第一次插入5V适配器充电时，瞬间SYS被击穿，对地短路。

这时芯片依旧能充电，但充电电流只有110mA左右。

另外8片目前正常。原理图如下：

负载峰值在1.7A左右，请帮忙分析一下可能的原因和对策。

目前10片烧了两片，不敢量产。
