C355,C356,C348电容建议改为0.22uF, C350,C353改为1uF.
请问你的芯片来源是？ 建议你找长期合作的渠道商或者直接找TI买芯片，如果是来自电子市场或者淘宝网站之类是很容易出现芯片质量问题的。
从上面的描述以及失效模式看，只要负载没有问题（推荐20mA内），很可能芯片本身有问题，所以建议确认购买渠道。
