您好，

      在贵司网站购买的PO:T01527280 LM340SX-5.0  3017pcs

      使用了356PCS，上线150件PCBA（单机1pcs），发现三例失效：2V转5V故障，实际输出只有2V。更换不良品后，电压恢复正常。（其他PCBA暂时未发现问题）

      要求：对不良的3例不良做出失效分析并提供相关报告。请提供不良品收货地址，谢谢。
