有一片REF3030AIDBZ在使用中出现失效问题，输入电压为5V，但输出电压也是5V，输入限流电阻为22R，电路图如下。请问什么原因可能导致芯片失效。

测试一下芯片输入输出是否击穿？ 很可能芯片已经损坏。
