TPS54295: 工作时突然输出失电

我采用TPS54295作为电源管理芯片，输入：5V，输出：3.3V和1.2V。现在遇到的问题是工作时tps54295的3.3V和1.2V有时会突然失电，失电后其输入5V还是正常的。而且要断电后重新上电，3.3V和1.2V才能恢复正常。想请教一下，什么原因可能导致这种情况？
