For OPA454, the maximum E/D to E/D Com Voltage is 5V. It is NOT recommended to connect E/D to V+ through 200Kohms. Besides, it is NOT recommended to float amplifier input even if this amplifer is NOT used.
So please try to adjust your E/D voltage, and ground the input through pull-down resistor (if possible) to check if the circuit is ok? thanks.
