Hello,
1) The Pin5 of datasheet is also MUTE/, not EN pin. www.ti.com/.../drv632
2) The attached application circuit is similar with Figure 9, which could be used as the differential right channel input and output single-end right channel, the differential left input and single-ended left channel.
In addition to the application typic circuit of Figure9, you could also refer to the EVM user's guide of Page8:
