1. Transient protection is missing. We recommend using TVS diode at the input and Schottky diode at the output to help against transients which are generated in events like hot-plug or short circuit and can cross the absolute maximum of the device resulting in permanent damage to device.
2. Can you check the EN pin connection , if you want to use it , you need to connect the resistor ladder from VIN pin to gnd.
3. The PG pin is recommended to pull up with resistor of value 47K ohm
4. Connecting the IMON pin to ground disables the active current limit protection during startup
