AD直流电压采样

我用LV25P来采样12V直流电源，经调试LV25P的M输出4.5V正常，但是后面的电压跟随器的Uo位置是13.6V，并没有跟随4.5V。请问是哪里出现了问题，还是我跟电压跟随器设计的不对呢？新人求解。放大芯片用的是OP27GSZ。
