Fault 1011 is an indication of bg_good going low (table 3 of the data sheet) unexpectedly. From figure 4 you can see the dependencies are VDDCP, the Cpgood signal, 12V or internal reference damaged. You might check for a missing or high impedance power pin connection, or open or short which keeps the charge pump from operating properly. If it shows up when only certain switches are selected perhaps it could be a short on those gate drivers. If the board circuit is correct, and well connected, it may be a damaged part.
