UCC81461 RCM locked issue (for Ericsson)

目前我们的客人在使用UCC81461时发现了锁死状态，需要将RCM接GND激活，然后可以正常使用。 我按照datasheet 模拟了上面说的OTP， UVP和UCCP都不能出现锁死状态，请问一下这个IC怎样能进去锁死状态，我们使用过程中，或者设计中，如何避免锁死？谢谢！
