你们好，我司现使用TI的DC DC降压电源TLV62130，电路如

输入为12V，输出3.9V接GSM模块，现在出现在一个2000批量中出现十几台在使用过程中电源芯片TLV62130烧毁的情况，如下图所示：

出现这种情况下,GSM模块也烧损，4V的输入端出现短路，请问是什么情况会出现这么严重的烧毁现象？
