BQ25792: damage issue

您好，The schematic looks okay.  The layout does not follow the datasheet and EVM recommended layout but that should not result in damage, only output power limitation.  Most likely, the cause is from 4S battery pack hot plug.  Is there a specific event, like battery attach or remove, that damage occurs?  Is the transient suppressor diode D2 damaged as well?  

Thanks for your comment, so far we haven't found a path to replicate the fault, it's random.
D2 doesn't appear to be damage.anyway, I'll check again.
