UCC28064A: 客戶使用在無載115Vin的時候，兩側的mos and Rcs會機率性燒毀

Hi team,

我是TI analog FAE located在台北。如標題所述，我客戶在使用UCC28064A時於客戶使用在無載115Vin開機的時候，兩側的mos and Rcs會機率性燒毀,目前是 燒率/所有樣品版= 7/260的機率。想請教是否有遇過類似的狀況或者有可能發生的原因?有檢查過Rgs=10k ohms。目前手上有客戶的schematic，如果OK煩請email給我做更進一步的debug。
