BQ24170充电电路的PWM问题

调试了好几块电路板，都出现相似的问题，i电池充电几分钟后，BQ24170就烧坏了。

表现有两种：

1、sw信号和PGND导通了

2、sw信号输出为固定13.8v电压，充电输入电压为15v

哪位能帮着分析一下，是什么问题

ps：电路时参考datasheet搭的
