1) Coupling cap value is chosen based off the high pass filter formed by the AC cap + input impedance R.
2) yes you are correct. Refer to table 2-1 for the table on jumper configurations for various scenarios.
3) Schematic looks okay. Remember for layout to connect the MICBIAS star as close to the device pin as possible to reduce any cross-talk through the MICBIAS node.

The AC coupling cap is required on both legs as well as the MICBIAS resistor to provide bias to the ADC input pins. Check the diagrams in the data sheet for references on this.There is a link in the datasheet to a tool that will help you calculate R1.
