您好，A short on Clamp 1 and Clamp 2 is indicating an over voltage condition on the input.

The circuit looks OK

Couple of things:

1.) Send data sheet on the coil for review

2.) Check voltage rating on capacitors on AC side, should be min of 25V.

3.) When does the fail occur? 

Moving the RX from a weak coupling position to a good position will cause a voltage spike on input and possible damage.

How much load in on the Coil,  250mA to 500mA?

CLAMP 1 / 2 are often active when the device is moved from a Poor coupling position to a Good coupling position.  The AC 1 / 2 signal will go from  normal to very high.

Have the fails been seen during device movement?
